Chronic Care Management (CCM) supports patients with chronic conditions in the community where they can be followed up on a regular basis throughout each year. The programme gives patients with a chronic condition the opportunity to work with their general practice team to help improve the management of their condition. The CCM programme delivers a continuum of care by integrating various services. CCM helps facilitate changing provider behaviour from acute episodic care to structured case management.

Providing care for the treatment of severe conditions that are of short duration and at a crisis level, where patients can be managed within the community and in Primary Care.

+ Child & Youth Health >>

Good child health is important not only for children and families now, but also for good health later in adulthood. Good health is essential to the positive development of young people. Generally, most young people are healthy most of the time. Health is holistic and the health of young people is often a reflection of what is happening in their lives – positive connections with family / whānau, school, peers, communities and work and training are all important for healthy development.

Women’s Health is aimed at encouraging women from all walks of life to be more aware of their health and to access healthcare.  Women's Health covers a wide range of issues, from Pregnancy and Menopause to STDs and Contraception. This includes providing targeted women’s health checks at convenient times and locations, reproductive care, education and health promotion on women’s health issues made available.

Men’s Health is aimed at encouraging men to be more aware of their health and to access healthcare. This includes providing targeted men’s health checks at convenient times and locations, supporting workplace based men’s health initiatives, establishing a men’s health innovation group to support community based men's health initiatives, improving access to good quality men’s health information.

In order to provide modern state of the art care further research is needed to provide innovative and best management approaches to care. By doing this, Primary Care will continue to develop and look for evidence based best practice avenues for better, sooner and more convenient care.

+ Mental Health >>

Mental health is not just the absence of mental disorder. It is a state of well-being in which every individual realises his or her own potential, can cope with the normal stresses of life, can work productively and fruitfully, and is able to make a contribution to her or his community. The focus is on identifying barriers to health service use and the promotion of mental health throughout the lifespan to ensure a healthy start in life for children and to prevent mental disorders in adulthood and old age.

Empowering people to take control of their health and its determinants, and thereby improve their health. It is by providing health information and education to individuals, families, and communities that encourage family unity, community commitment, and traditional spirituality that make positive contributions to their health status. Health Promotion & Education involves a holistic approach to healthcare and well being.
